S. aureus méti-S

Traitement initial

  • Cefazoline IV 150 mg/kg/j en 4 fois par jour
    • Pas de dose max si fonction rénale normale, mais dosage à réaliser si > 8g/j 

OU

  • Cloxacilline IV 200 mg/kg/j en 4 fois par jour (max 12g/j) 

Si allergie aux β-lactamines

  • Clindamycine IV 40 mg/kg/j en 3 fois par jour (max 900 mg x3/j)

 

Relais PO

  • < 6 ans : Augmentin PO 80 mg/kg/j en 3 fois par jour (max 3g/j) 
    OU Cefalexine PO 150 mg/kg/j en 3 fois par jour (max 6g/j)
  • > 6 ans : Clindamycine PO 40 mg/kg/j en 3 fois par jour (max 900 mg x3/j)

Si allergie aux β-lactamines :

  • 6 mois – 6 ans : Cotrimoxazole PO 60 mg/kg/j en 2 fois par jour (max 1600 mg x2/j)
  • ≥ 6 ans : Clindamycine PO 40 mg/kg/j en 3 fois par jour (max 900 mg x3/j)

 

Kingella kingae

Traitement initial

  • Amoxicilline IV 100 mg/kg/j en 4 prises par jour (max 4g/j)

Si allergie aux ß-lactamines : avis infectiologique 

 

Relais PO

  • Amoxicilline 80 mg/kg/j en 3 prises par jour (max 3g/j) 

Si allergies aux β-lactamines :

  • Cotrimoxazole PO 60 mg/kg/j en 2 fois par jour (max 1600 mg x2/j)
    OU Ciprofloxacine 40 mg/kg/j en 2 fois par jour (max 750 mg x2/j)

 

S. pyogenes (groupe A), S. agalactiae (groupe B), S. pneumoniae (CMI impérative)

Traitement initial

  • Amoxicilline IV 150 mg/kg/j en 4 fois par jour (max 12g/j) 
    OU Cefotaxime IV 200 mg/kg/j en 3 fois par jour (max 12 g/j (<15 ans), 24g/j (> 15 ans)) 
    OU Ceftriaxone IV 75 mg/kg/j en 1 fois par jour (max 2g x2/j)

Si allergie aux ß-lactamines : avis infectiologique 

Relais PO

  • Amoxicilline PO 80-100 mg/kg/j en 3 fois par jour (max 6g/j) 

 

S. aureus meti-R

Traitement initial

En l’absence d’antibiogramme complet : 

  • Vancomycine IV 60 mg/kg/j en 4 fois par jour, ou en IV continue avec dose de charge de 15 mg/kg sur une heure (dosage à H48)
    + Clindamycine IV 40 mg/kg/j en 3 fois par jour (max 900 mg x3/j)

En cas d’insuffisance rénale : Linezolide IV 30 mg/kg/j en 2 (≥ 12 ans) ou 3 (< 12 ans) fois par jour (max 600 mg x2/j) 

Adaptation dès que antibiogramme complet : avis infectiologique

 

Relais PO : avis infectiologique

 

Neisseria gonorrhoeae, Neisseria meningitidis

 

Traitement initial

  • Ceftriaxone IV 75 mg/kg/j en 1 fois par jour (max 2g x 2/j)

Si allergie aux β-lactamines

  • Ciprofloxacine IV 30 mg/kg/j en 2 ou 3 fois par jour (max 750 mg x2/j)

Durée

  • 7 jours IV minimum

 

Salmonella sp

 

Traitement initial

  • Cefotaxime IV 300 mg/kg/j en 3 fois par jour (max 12 g/j (<15 ans), 24g/j (> 15 ans)) 

Si allergie aux β-lactamines: avis infectiologique

 

Relais PO

  • Ciprofloxacine 45 mg/kg/j en 3 fois par jour (max 1500 mg/j)
